Transcripts
Domestic applicants should request that an official transcript of your academic record from each college, university, or graduate and professional school you have attended be sent to your proposed program of study office. It is not necessary, however, to submit a transcript covering work completed at the Urbana-Champaign campus of the University of Illinois. When completing the application form, be sure to list all of the institutions you have attended, including the University of Illinois at Urbana-Champaign.
