Immunization Requirement
at Registration
Illinois state law requires all new students to provide proof of immunity to certain contagious diseases (among them: diphtheria, measles, mumps, rubella, and tetanus) before enrollment. All admitted students will receive an Immunization Information Form, which must be completed and submitted upon arrival on campus. All international students will be screened for tuberculosis at the time of enrollment in accordance with the Illinois Department of Public Health. All students please note: if the required Immunization Information Form is not submitted, you will not be eligible to register for future terms. Immunization Information Forms are also available online from the McKinley Health Center.
