Appendix A: Checklists

Checklist for Master's Degree Candidates

  1. Apply for graduation before deadline
    • Master’s students graduate in May, August, October, and December. In order to graduate, you must be on the pending degree list for the term you intend to deposit. You can add your name to the list by submitting the “Application for Degree” form or by using the UI Integrate Self-Service system before the 10th day of instruction. Keep in mind that the application does not carry over to the next graduation term. Therefore, if you do not deposit in time you must submit the form again. There is no penalty if you need to re-submit the form.
  2. Title page check in the Thesis Office
    • This step is essential to ensure that your name and previous degree information appears exactly as it is in your records. This page is edited with stringent guidelines, so refer to the ‘Title Page Format’ directions in the thesis handbook. Please come to the office early in the process to have this reviewed, so you have time to make changes if they are needed.
  3. Departmental Format Review
    • Your department may have their own deadlines and format rules. Check with them to ensure a timely review. The Departmental Format Approval form must not contain any white-out, and your name and title of thesis must be exactly as it appears in your records.
  4. Certificate of Committee Approval Form
    • A minimum of two signatures is required, your department may require additional. This form must not contain any white-out, and your name and title of thesis must be exactly as it appears in your records. The date should be the day, month, and year that the thesis was approved by your adviser (or of the oral examination, if applicable). This form must contain the student’s UIN in the upper right-hand corner.
  5. Format Review in the Thesis Office
    • No appointments are given; first-come are first-served. Your thesis will be checked, and you will be given a lavender sheet with necessary changes. You must bring this lavender sheet with you whenever you come into the office.
  6. Deposit the following materials before the deadline:
    • Two unbound, single-sided copies
    • Two letter-sized accordion pocket folders
    • Two extra title pages
    • One signed original Departmental Format Approval form
    • One original Certificate of Committee Approval form signed by department head and adviser
    • The $20 deposit fee will be charged to your student account. The Thesis Office no longer accepts checks.
    • Graduate College Survey form (optional)
  7. Update address information on UI-Integrate Self Service. Your diploma will be mailed to your DIPLOMA address.

Checklist for Doctoral Degree Candidates

  1. Register for 599 during semester in which defending
  2. Defend before deadline
  3. Apply for graduation before deadline
    • Doctoral students graduate in May, October, and December. In order to graduate, you must be on the pending degree list for the term you intend to deposit. You can add your name to the list by submitting the “Application for Degree” form or by using UI Integrate Self-Service before the 10 th day of instruction. The application does not carry over to the next graduation term; therefore, if you do not deposit in time you must submit the form again.
  4. Title page check in the Thesis Office
    • This step is essential to ensure that your name and previous degree information appears exactly as it is in your records. You must adhere to the ‘Title Page Format’ directions in the Thesis Handbook. Please come to the office early in the process to have this reviewed, so you have time to make changes if they are needed.
  5. Departmental Format Review
    • Check with your department regarding their own deadlines and format rules. The Departmental Format Approval form must not contain any white-out, and your name and title of thesis must be exactly as it appears in your records.
  6. Certificate of Committee Approval Form
    • This form must have a total of at least six signatures. Even if the same person filled dual roles (e.g., Head of Department and on committee), s/he must sign twice. At least four signatures are required in the Committee section. This form should not contain any white-out, and your name and title of thesis must be exactly as it appears in your records. The date should be the day, month, and year of your defense. This form must contain the student’s UIN in the upper right-hand corner.
  7. Format Review in the Thesis Office

  8. Deposit the following materials before deadline:

    • Three extra title pages
    • One original Departmental Format Approval form, signed
    • One original Certificate of Committee Approval form, signed by entire committee
    • ProQuest/UMI Publishing Agreement form, signed
    • One abstract, formatted according to ProQuest guidelines
    • Survey of Earned Doctorates
    • The $90 deposit fee will be charged to your student account. The Thesis Office no longer accepts checks. This fee will be increasing for May 2008 graduates.
    • Graduate College Exit Survey form (optional)
  9. Update address information on UI-Integrate Self Service. Your diploma will be mailed to your DIPLOMA address.
